The table below contains a list of relevant forms necessary for application to the West Virginia Board. Applicants must read the WV Laws, Rules and Regulations for the engineering profession for which they are requesting licensure. Click here for information on the WV Laws, Rules and Regulations for this Board. You must have the Adobe Acrobat Reader in order to view and print the Adobe Acrobat PDF files or you may use Microsoft Word to open and complete the Word forms. If you do not have the Adobe Acrobat Reader program installed on your computer, you may obtain a free copy by clicking on the "Get Acrobat Reader" link at the bottom of this page. The WV State Board of Registration for Professional Engineers will only accept application forms that print out exactly as the ones provided on this site. If the forms do not print out on your printer EXACTLY as they appear on the screen, contact the WV Board office to request hard copies be sent to you through postal mail.
